Service Population:

Openings in each of the D & J Home Care, Inc. adult foster homes are available to all adult males and females, who are 18 years of age or older and developmentally disabled, mentally ill or aged. Referrals may be made through Mental Health agencies or private individuals.

Objectives:

To provide room and board in a safe, comfortable and pleasant home environment for up to six developmentally disabled, mentally ill or aged adults.

To offer assistance with the activities of daily living (ADL) until maximum levels of skills are set, attained and maintained. Through daily observation, to offer general supervision of health status on a twenty-four hour basis, seven days a week.

To oversee the administration of prescribed medications and diets.

To treat all individuals equally, regardless of race, religion, sex, color, or national origin.

SUNNY HAVEN

LOCATION:

The Sunny Haven AFC group home is situated on a quiet residential street in the city of New Haven, in Macomb County, Michigan.

Close to all city conveniences, Sunny Haven is within walking distance to the Public Library, several churches and the city park. Within a few minutes drive are: movie theatres, shopping, bowling, restaurants and a host of other community activities to enjoy.

CHESTERFIELD HOME CARE

LOCATION:

The Chesterfield AFC group home is conveniently located in the fine community of Chesterfield Township in Macomb County, Michigan.

Situated in a quiet subdivision, this six-resident home is near shopping, recreation, churches and public transportation. The home is also within a few minutes drive to movie theatres, bowling, restaurants and other community activities.

D & J HOME CARE

LOCATION:

The D & J AFC group home is located off of 26 Mile Road in peaceful Chesterfield Township in Macomb County, Michigan.

Surrounded by beautifully wooded scenery, this six-person residence sits on 3.5 acres with the "Deer Creek" gently flowing through it's landscape. This warm and inviting home is within a few minutes drive to shopping, churches, movie theatres, bowling, restaurants, and many other forms of recreation within the community.

LOCATION:

The Haven Hills AFC group home is situated on a quiet residential street in the city of New Haven in Macomb County, Michigan.

Close to all city conveniences, Haven Hills is within walking distance to the Public Library, several churches and the city park. Haven Hills is also within walking distance to City Hall and the New Haven Police Department. Within a few minutes drive are: movie theatres, shopping, bowling, restaurants and various other community locations.
